From U.S. Department of Health & Human Services As Alternatives to Nursing Home Care Medicare offers limited access to programs for beneficiaries who need a comprehensive medical and social service delivery system. A Social Managed Care Plan, available in only certain states, provides in addition to Medicare benefits services which include care coordination, prescription drug benefits, home and community based services such as homemaker, personal care services, adult day care respite care, and medical transportation.
