This site is a good resource in understanding the need to better coordinate and manage medical care, which has given rise to Johns Hopkins’ proposal for “Guided Care Nurses.” However, since Medicare does not yet pay for the services of the proposed “guided care nurse,” in practice, family caregivers will need to learn to coordinate and manage the care of their chronically ill elderly loved ones, unless they can afford to pay for a private care manager or “guided care nurse” to help them.
