The National Hospice and Palliative Care Organization is a non-profit organization that aspires to improve care at the end of life through its Caring Connections project, a national consumer and community engagement initiative, originally funded in part by the Robert Wood Johnson Foundation. Caring Connections works to ensure that individuals have access to information that informs advance care planning, care at the end of life and the grief process.
You should consult a competent attorney with experience in the use of Health Care Advance Directives in your state before using any of these forms.
